Advancing Communities Everywhere is seeking to immediately hire 1-3 Full or Part-Time Licensed Psychiatric Nurse Practitioners to implement Medical Health services as an integrated part of the Crisis Care Continuum Model in Community Mental Health Crisis services. The ideal Licensed Psychiatric Nurse Practitioner will provide medical health care services to include, vitals, medical health literacy education, and medical health exams to Advancing Communities Everywhere participants in 23-Hour Emergency Crisis Services. Additionally, Licensed Psychiatric Nurse Practitioner will also be responsible for completing the required Psychiatric Evaluation and Medication Management Evaluations for our 23-Hour Crisis Stabilization program participants. TheLicensed Psychiatric Nurse Practitioner will work with identified individuals who are experiencing acute and severe mental health conditions in the community setting.
23-Hour Crisis Stabilization provides ongoing assessment, crisis intervention and clinical determination for level of care to individuals experiencing a behavioral health crisis. Services are provided for a period of up to 23 hours in a community and center-based crisis stabilization setting including outpatient hospital settings that have an Outpatient Crisis Stabilization license. This service must be accessible 24/7 and is indicated for those situations wherein an individual is experiencing a behavioral health crisis and requires a safe environment for observation and assessment prior to determination of the next level of care. Although not required, 23Hour Crisis Stabilization services typically co-locate with RCSUs as part of a continuum of crisis care. 23-Hour Crisis Stabilization is appropriate for individuals who have urgent behavioral health needs including but not limited to significant emotional dysregulation, disordered thought processes, substance use and intoxication resulting in behavioral crisis and environmentally de-stabilizing events that require multi-disciplinary crisis intervention and observation to stabilize the immediate crisis and determine the next appropriate step in the plan of care.
JOB DESCRIPTION-
Responsibilities:
- Provide case management services to patients, coordinating their care and ensuring their needs are met
- Conduct patient assessments, including vital signs, medical health exam, diagnostic evaluations and Patient Health Literacy
-Conduct Psychiatric Evaluations for patients displaying Mental Health SMI.
-Conduct Medication Management screenings, and prescribe or reccomendations for psychotropic medications for patients displaying Mental Health SMI.
- Develop and implement care plans based on patient needs and goals for Medical Health Care
- Collaborate with healthcare team members and MH Care Team to ensure continuity of care
- Educate patients and their families on medical conditions, treatment options, and self-care techniques
- Maintain accurate and up-to-date patient records in compliance with HIPAA regulations
- Monitor patient progress and adjust care plans as necessary
- Coordinate referrals to specialists and other healthcare providers as needed
-Nurse Practitioners shall hold an active license issued by the Virginia Board of Nursing. RNs and LPNs shall hold an active license issued by the Virginia Board of Nursing or hold a multistate licensure privilege pursuant to Chapter 30 (§ 54.1-3000 et seq.) of Title 54.1 of the Code of Virginia.
